SummaryWe are seeking a dedicated and experienced Senior LTSS Service Care Manager (RN) to support pediatric members with long‑term services and supports (LTSS). This role is responsible for conducting comprehensive member assessments, coordinating care needs, and ensuring continuity of services for members transitioning from prior managed care organizations (MCOs). This is a highly collaborative, member‑focused team environment where accountability, flexibility, and strong clinical judgment are essential to success.
CompensationThe pay for this position is $40/hr + mileage. Benefits are available to full‑time employees after 90 days of employment.
HighlightsThis position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs.
ScheduleMonday–Friday, 8:00 AM–5:00 PM. Overtime possible and occasional Saturdays.
Responsibilities- Prepare for member visits by reviewing diagnoses, medications, recent authorizations, and electronic medical record documentation.
- Conduct in‑person member assessments including initial assessments, change in condition visits, MDCP assessments, and Level 3 reassessments.
- Travel to member homes for visits lasting approximately 2–4 hours.
- Travel may require up to 2 hours each way.
- Submit completed assessments within 24 hours.
- Initiate service requests within 3 business days.
- Complete all required documentation within 5 business days.
- Transition members to permanent care management teams within 15 business days.
- Complete required Centene University trainings by assigned deadlines.
- Manage assigned tasks and workflows within established turnaround times.
- Active Registered Nurse (RN) license.
- ASN, BSN, or MSN required.
- 3+ years of nursing experience.
- 2+ years of case management experience.
- Strong clinical judgment and critical thinking skills.
- Ability to prioritize workload and demonstrate accountability.
- Ability to take direction and work independently.
- Pediatric experience strongly preferred.
- Experience with LTSS, STAR Kids, or managed care environments preferred.
