The GN Graduate Nurse, under the supervision of RN, will assist in ensuring the provision of quality care that promotes the highest practicable physical, mental and psychological well-being of each resident as determined by resident assessments and individual plans of care.
Essential Functions- Observe, assess, document, and report resident conditions/changes during rounds.
- Assure implementation of care plans as directed.
- Administer medications as prescribed.
- Conduct rounds with physician and record visits in clinical records.
- Perform narcotic reconciliation with incoming and outgoing shifts.
- Identify significant changes in the condition of residents and take necessary action per facility policies and procedures.
- Oversee and monitor aides.
- Initiate and complete Incident and Accident Reports per facility policy.
- Discharge residents per facility policy.
- Complete Nursing Assessment for Physician Calls prior to calling physician on any change in resident condition.
- Communicate with resident’s point of contact when they ask for a status update or if there is a change in condition.
- Participate in other special projects or functions as assigned by the Director of Nurses and/or the Administrator.
- Completed an RN program at an accredited vocational school or college.
- Must be working toward testing for a Registered Nurse license in the state of Texas.
- Experience working in a skilled nursing facility is preferred.
- Lifting 1-25 lbs – performed rarely to frequently.
- Lifting 25-50 lbs – performed occasionally.
- Lifting 50+ lbs – performed frequently.
- Carrying 1-25 lbs, 25-50 lbs, & 50+ lbs – may carry up to 50+ lbs.
- Pushing/Pulling 1-25 lbs, 25-50 lbs, & 50+ lbs – may push or pull up to 50+ lbs.
- Sliding/Transferring 1-25 lbs, 25-50 lbs, & 50+ lbs – may slide or transfer residents.
- Standing, Sitting, Walking – may spend long periods standing or walking.
- Speaking, Hearing/Listening, Seeing – required for communication.
- Balancing/Climbing, Stooping/Kneeling, Crouching/Crawling, Reaching – required for resident care.
- Driving – required for transportation of residents.
