Overview
FNP Per‑visit role, 8 hrs per week (you build your schedule); $145.00 per home visit (includes mileage) and $85.00 per telehealth visit. Medicare/Medicaid enrollment required to conduct Annual Risk Assessments with geriatric population and Chronic Condition Verifications with pediatric population.

The Nurse Practitioner delivers annual risk assessment in a residential setting or telehealth, within the scope of practice for a Nurse Practitioner, as delegated by the Collaborative Physician.
- Conduct comprehensive in‑home health risk assessments to identify all active and chronic disease conditions, and determine all physical, mental, and social needs present at the time of the visit.
- Take history, examine, determine diagnoses.
- Provide written documentation of patient visit, per NCQA standards.
- Take patient vital signs as necessary.
- Place case management referrals and communicate with PCP as necessary.
- Communicate with patients, caregivers, agency nurses, other providers and vendors as necessary to assure proper diagnosis.
- Perform all clinical duties while observing OSHA Universal Precautions.
- Maintain patient confidentiality.
- Attend required meetings and in‑services and participate in committees, as requested.

- Active/unrestricted nurse practitioner license to practice in coverage states.
- Board certification in ANCC, AANP or NCCPA.
- Active BLS certification.
- Current enrollment in Medicare/Medicaid.
- Valid driver’s license and good driving record.
- Outstanding EHR skills.
- Geriatric training/experience.
- Skill in teamwork and maintaining effective working relationships with patients, medical staff, and the public.
- Adaptability to differing weather conditions and patients’ home/residential environments.
- Full range of body motion including handling/lifting patients. Manual and finger dexterity, eye‑hand coordination, normal visual acuity, normal hearing, standing, bending, walking and stair climbing.
- Regular lifting/carrying items weighing up to 50 pounds.
- Ability to ride in automobile or van up to 150 miles daily in urban and/or rural settings; ability to drive if necessary.
