Grace Unlimited Home Health Care is seeking experienced PRN RN Case Managers to provide high-quality home healthcare to clients primarily in the Fort Worth area, with coverage in surrounding Dallas–Fort Worth communities as needed. This role is ideal for nurses seeking flexible PRN work while delivering compassionate, patient-centered care in the home.

• Provide professional, compassionate nursing care in clients’ homes
• Perform assessments and manage individualized care plans
• Accurately document care using OASIS/Kinnser
• Manage diverse patient needs in a home health setting
• Respond to PRN and urgent visit needs as assigned

• Graduate of an accredited nursing program
• Active Texas RN License
• Valid Driver’s License and reliable transportation
• Minimum 1 year of home health experience
• Experience with OASIS/Kinnser preferred
• Ability to travel locally within the Fort Worth area
• Flexible availability for PRN visits

• Client homes throughout the Denton area
• Field-based / on-the-road role
