Overview

The Value-Based Care (VBC) Nurse Practitioner provides patient-centered clinical care and care management for individuals with kidney disease, collaborating with nephrologists, dialysis care teams, and other providers to support quality outcomes, reduce hospitalization risk, and promote effective management of chronic and comorbid conditions.

Responsibilities

SUMMARY

Essential Duties and Responsibilities include the following. Other duties and tasks may be assigned.

Primary duties include:

  • Holistic patient care management including but not limited to engaging patients, screening for depression, addressing risks of hospitalization, providing transitions of care following hospitalizations, and supporting management of kidney health and comorbid conditions.
  • Collaborates with nephrologists, dialysis center interdisciplinary teams, Kidney link colleagues, hospital discharge care coordination team, primary care physicians and sub-specialists as clinically indicated to address patient care needs.
  • Builds and manages close partnerships with nephrologists and dialysis team members, as appropriate, to manage care.
  • May provide comprehensive disease management annual visits
  • May include dialysis rounding, including limited and comprehensive visits, under the guidance of partner practice nephrologists
  • May involve up to 100% of time spent in USRC dialysis clinics, partnering with dialysis teams and nephrologists to manage care.
  • Other duties and tasks as assigned

Functions performed during above duties:

  • Perform physical examinations of patients to evaluate health and diagnose illnesses
  • Order and assess diagnostic tests, such as x-rays and laboratory tests
  • Determine, plan, and initiate appropriate treatment, and adjust treatment if necessary
  • Prescribe medications
  • Document medical information of patients and maintain electronic medical records
  • Educate, instruct, and counsel patients and families on presenting health problems, treatment plans, and home care, and encourage and promote preventative health care and positive health behaviors
  • Request consultation or referral with other health care providers, such as physicians and clinics
  • Collaborate with other members of the Kindeylink / USRC clinical team
  • Collaborate with practice partner physicians and other providers
  • Actively promotes GUEST customer service standards; develops effective relationships at all levels of the organization.
  • Participates in team concepts and promotes a team effort; performs duties in accordance with company policies and procedures.
  • Regular and reliable attendance is required for the job.
Qualifications

Qualifications/Requirements:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.

Requirements include:

  • Current Advanced Practitioner license in Texas. License must be maintained as current and in good standing.
  • Three (3) plus years of experience preferred.
  • Previous experience in direct patient care to dialysis patients and/or case management preferred.
  • Demonstrated working knowledge of the English language and ability to communicate verbally and in writing, Spanish/Bi-lingual a plus.
  • Must have basic computer skills; proficiency in all USRC clinical applications required within 90 days of hire.
  • Must meet any practice requirement(s) for the applicable state.
  • Ability to meet minimum hospital/agency hiring standards which may include additional background clearances and orientation requirements if applicable to the program assigned to for employment.
  • Candidate must reside in Laredo.
