Human Services Counselor III

This is an internal promotional opportunity. The Human Services Counselor III is a home visiting Family Support Specialist utilizing trauma‑informed practices to support and provide assistance to high‑risk pregnant women and families with newborns to optimize the home environment for their physical and mental health well‑being. The role empowers families with the tools and resources to break negative intergenerational cycles and is part of a child abuse/neglect prevention program.

Specific Responsibilities
  • Establish a trusting relationship with multifaceted families by initiating and maintaining regular client contact as prescribed by Healthy Families America accreditation standards.
  • Provide case management and care coordination for pregnant women and families of infants by visiting their homes for a period of up to 5 years.
  • Serve as the primary coordinator on a multi‑disciplinary team including therapist, nurses, and father support specialists.
  • Maintain a case weight according to program requirements and meet Healthy Families Pinellas guidelines for home visiting and risk‑appropriate care.
  • Link pregnant women, families, and infants to community supports and services.
  • Assure continued participation in prenatal and infant health care and communicate with healthcare providers.
  • Track prenatal care, immunizations, and well‑baby care; close cases in a timely manner when no further services are needed.
  • Ensure services satisfy the needs of the participant and act as a mandated reporter.

As a Human Services Counselor III, act as a team lead. Lead responsibilities may include but are not limited to training new home visitors, planning and implementing parent events, recruiting families for QA calls, representing the program at community events, attending and presenting at management meetings, and participating in QA activities. May manage bus passes and other client incentives in the supervisor’s absence and serve as a general backup.

In The Home
  • Determine participant’s needs through screenings and assessments including the initial home assessment, home safety questionnaires, HFPIs, CCIs, Edinburgh Depression Screens, and developmental screening tools.
  • Assist the family in developing Family Goals and strategically implement curriculum and referrals on the HVSP to address risk factors.
  • Capitalize on protective factors and facilitate the development of additional protective factors within the family.
  • Implement reflective strategies to promote attunement within the parent‑child relationship.
  • Provide in‑home parent education utilizing recommended curriculums such as GGK, the Gold Within, and Just in Time parenting curriculums.
  • Provide women/parents with family planning options and education on planning future healthy deliveries with an emphasis on women’s health.
  • Offer tobacco risk and smoking cessation education, counseling, and referrals.
  • Utilize Healthy Families Pinellas forms to document case management, client progress, and home visiting activities.
  • Maintain an electronic health record for each client and complete service ledgers in the HFF data tracking system.
  • Complete Encounters and EARS in HMS and all HVRs within program standards.
  • Meet grant goals, performance measures, and funder requirements; maintain standards and applicable regulations.
  • Meet weekly with the supervisor for a minimum of 1.5 hours for reflective, clinical, and administrative supervision.
  • Participate in regular staff meetings and the implementation of parent groups and other participant events.
  • Attend training to promote professional and personal growth and complete all mandatory trainings as required.
