Bay Care is currently in search of our newest Home Health Aide Per Visit who is passionate about providing outstanding customer service to our community. We are looking for an individual seeking a career opportunity with one of the largest employers within the Tampa Bay area.

Full-Time or PRN
- Per Visit, hourly - Shift
Hours:
7:00 AM - 7:00 PM, Varies - Weekend Work:
Every 3rd weekend - On Call:
No
Key Responsibilities
- As part of the patient care team, works under the guidance and supervision of the staff nurse in providing personal care to the patients
- Follows the patient's plan of care for completion of tasks assigned to a home health aide by the registered nurse or other appropriate skilled professional
- Maintains an open communication process with the patient, representative (if any), caregivers, and family
- Demonstrates competency with assigned tasks
- Complies with infection prevention and control policies and procedures
- Reports changes in the patient's condition
- Honors patient rights
- CNA (Certified Nursing Assistant) or HHA certification from accredited 75-hour program
- Driver's License State of Florida (Florida driver's license is required within 30 days of hire)
- BLS (Basic Life Support)
- Preferred:
High School Diploma or equivalent
- Preferred 1 year Home Care
This position requires Level II screening through the Florida Care Provider Background Screening Clearinghouse, managed by the Agency for Health Care Administration (AHCA). More information can be found at
