HUMAN SERVICES COUNSELOR III

Your Specific Responsibilities:

* Establish a trusting relationship with culturally diverse families by initiating and maintaining regular client contact.

* Provide Case Management to pregnant women and infants referred to the Healthy Start Program.

* Provide face to face contact with clients either in the OB provider's office or in the client's residence.

* Provide Parenting Education one on one with clients.

* Provide Breastfeeding Education and Tobacco Cessation Education to individual clients.

* Document all client contact information in the web-based computer application known as the Well Family System

* Provide referrals to community resources to supplement Healthy Start services.

Required Knowledge, Skills, and Abilities:

* Fluency in English and Spanish

* Experience working with pregnant women and children.

* Problem solving skills related to advancing client self-sufficiency.

* Ability to work well with others as a team.

* Computer skills: competency and experience.

* Experience in interviewing and assessing to determine the needs of the client.

* Knowledge of community resources.

Qualifications:

Minimum
- * Valid Florida Driver's License

* Minimum of four-year college degree in one of the following areas;
Social Sciences; a health-related field such as Nursing, Health Education, Health Planning, or Health Care Administration; or Social Work as required by FL Healthy Start Standards and Guidelines (Chapter 6-2)

* At least 1 year experience working with high-risk pregnant women and/or families

* Must have reliable and dependable transportation every day for home visitation purposes

Preferred
- * 1 to 3 years of experience working with families

* 1 to 3 years home visiting experience

* 1 to 3 years case management experience

* Bilingual with fluency in English and Spanish
