Human Services Counselor III -
Your specific responsibilities:
- Establish a trusting relationship with culturally diverse families by initiating and maintaining regular client contact.
- Provide case management to pregnant women and infants referred to the Healthy Start Program.
- Provide face to face contact with clients either in the OB provider's office or in the client's residence.
- Provide parenting education one on one with clients.
- Provide breastfeeding education and tobacco cessation education to individual clients.
- Document all client contact information in the web-based computer application known as the Well Family System.
- Provide referrals to community resources to supplement Healthy Start services.
Required knowledge, skills, and abilities:
- Fluency in English and Spanish.
- Experience working with pregnant women and children.
- Problem solving skills related to advancing client self-sufficiency.
- Ability to work well with others as a team.
- Computer skills: competency and experience.
- Experience in interviewing and assessing to determine the needs of the client.
- Knowledge of community resources.
Qualifications:
- Minimum:
Valid Florida Driver's License, minimum of four-year college degree in one of the following areas; social sciences; a health-related field such as nursing, health education, health planning, or health care administration; or social work as required by FL Healthy Start Standards and Guidelines (Chapter 6-2), at least 1 year experience working with high-risk pregnant women and/or families, must have reliable and dependable transportation every day for home visitation purposes. - Preferred: 1 to 3 years of experience working with families, 1 to 3 years home visiting experience, 1 to 3 years case management experience, bilingual with fluency in English and Spanish.
