Working Title:

HUMAN SERVICES COUNSELOR III -

• Establish a trusting relationship with culturally diverse families by initiating and maintaining regular client contact.
• Provide case management to pregnant women and infants referred to the Healthy Start Program.
• Provide face-to-face contact with clients either in the OB provider’s office or in the client’s residence.
• Provide one‑on‑one parenting education to clients.
• Provide breastfeeding education and tobacco cessation education to individual clients.
• Document all client contact information in the web‑based computer application known as the Well Family System.
• Provide referrals to community resources to supplement Healthy Start services.

• Fluency in English and Spanish.
• Experience working with pregnant women and children.
• Problem‑solving skills related to advancing client self‑sufficiency.
• Ability to work well with others as a team.
• Computer skills: competency and experience.
• Experience in interviewing and assessing to determine client needs.
• Knowledge of community resources.

Minimum

• Valid Florida Driver’s License.
• Minimum of a four‑year college degree in Social Sciences, a related health field such as Nursing, Health Education, Health Planning, or Health Care Administration; or Social Work, as required by FL Healthy Start Standards and Guidelines (Chapter 6‑2).
• At least one year of experience working with high‑risk pregnant women and/or families.
• Reliable and dependable transportation for daily home visitation purposes.

Preferred

• 1 to 3 years of experience working with families.
• 1 to 3 years of home‑visiting experience.
• 1 to 3 years of case‑management experience.
• Bilingual with fluency in English and Spanish.
