Home Health Aide (HHA) - Private Duty - PRN

Lifecare Home Health -  - Responsibilities:
Supports a patient’s Plan of Care (POC) by monitoring and recording patient condition, providing support and personal services, and teaching families;
Monitors and assesses patient condition by observing physical and mental condition, intake and output, and exercise;
Performs patient assessments and collaborates with the multi-disciplinary care team to develop and implement a plan of care;
Contributes to the continuity of care through effective communication with the designated Care Team;
Maintains a safe, secure, and healthy environment by following asepsis and universal precaution controls in performing client care
