Senior Care Solutions is a Medicare-certified home health agency dedicated to providing high-quality, patient-centered care in the comfort of our patients' homes. We are looking for a compassionate and motivated Physical Therapist (PT) to join our growing clinical team.

The Physical Therapist will provide skilled physical therapy services to home health patients based on their individual needs and established plans of care. This position is ideal for a PT who enjoys building relationships with patients, working independently, and making a meaningful impact on patients' functional independence and quality of life.

• Provide skilled physical therapy evaluations and treatments to patients in their homes.
• Develop individualized treatment plans based on patient needs and physician orders.
• Perform assessments and establish appropriate goals for each patient.
• Provide therapeutic exercises, mobility training, gait training, balance activities, and other skilled interventions as appropriate.
• Educate patients and caregivers regarding treatment plans, home exercise programs, safety, and disease management.
• Monitor patient progress and modify treatment plans as needed.
• Communicate changes in patient condition and progress with the physician and interdisciplinary care team.
• Complete all required clinical documentation accurately and within required time frames.
• Coordinate care with RNs, OTs, STs, MSWs, and other members of the interdisciplinary team.
• Participate in case conferences and care coordination as needed.
• Maintain compliance with Medicare, state, and agency requirements.
• Provide exceptional patient care while maintaining professional standards.

• Current Florida Physical Therapist (PT) license in good standing.
• Graduate of an accredited Physical Therapy program.
• Current CPR certification.
• Previous home health experience preferred, but not required.
• Strong clinical assessment and documentation skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage a flexible field-based schedule.
• Reliable transportation and willingness to travel within the assigned service area.
• Comfortable using an EMR system for clinical documentation.
• Strong organizational skills and attention to detail.
