Assume responsibility for patient evaluation and complex discharge planning.
Coordinate patient care and communicate effectively with interdisciplinary teams.
Utilize Midas and Meditech for documentation and case management activities.
Provide clinical support and guidance to staff members.
Experience with Midas and Meditech.
Knowledge in complex discharge planning and patient care management.
Experience in a case management role within a hospital setting.
Strong communication and organizational skills.
