Physical Therapist In Home Health
Join our team as a Physical Therapist in Home Health, where you'll provide essential care to patients in their homes.
What You'll Be Doing- Conduct patient assessments in home health settings
- Utilize various equipment and devices for patient care
- Document patient progress and treatment plans
- Educate patients and families on home exercise programs and safety
- Perform skilled home care visits, averaging 6-8 per day
- Self-schedule visits based on office needs
- Active state license as a Physical Therapist
- Current BLS certification
- At least 1 year of home health experience
- Experience with patient assessments
- Valid driver's license and car for home visits
- OASIS training
