PRN Occupational Therapist (OT) – Home Health

We are seeking a compassionate and self‑motivated Occupational Therapist (OT) to join our Home Health team on a per diem (PRN) basis. In this role, you will provide individualized, in‑home therapy services to help patients regain independence in their daily living and self‑care activities. • Perform initial and ongoing assessments of patients’ functional abilities, ADLs, mobility, safety, and home environment.
• Develop and implement individualized care plans based on physician orders and patient needs.
• Deliver therapeutic interventions including training in self‑care, use of adaptive equipment, and home safety modifications.
• Educate patients and caregivers to support recovery and independence.
• Accurately document all care provided using Well Sky.
• Coordinate care and communicate effectively with the interdisciplinary home health team.
• Maintain compliance with agency, state, and federal guidelines.

• Current FL licensure as an Occupational Therapist (OT).
• Minimum 1 year of clinical experience; home health experience preferred.
• Familiarity with Well Sky EMR or similar documentation systems preferred.
• Strong clinical reasoning, time management, and communication skills.
• Ability to work independently and manage a flexible field‑based schedule.
• Reliable transportation, valid driver’s licence, and current auto insurance.
