Travel Physical Therapist - Home Health

American Traveler is seeking a Physical Therapist for a Home Health assignment requiring at least 1 year of home health experience and a valid state PT license.

• Minimum 1 year of experience required, including home health experience
• Current BLS certification required
• State PT license required if applicable
• OASIS training preferred
• Valid driver's license and personal vehicle required for home visits
• Proficiency with equipment including gait belts, rolling walkers, sit-to-stand lifts, sliding boards, soft/hard collars, standard walkers, supplemental oxygen delivery, upper extremity prosthetics, and wheelchair assessment
• Skilled in assessment, ortho and neuro treatment, cardiopulmonary/circulatory care, home exercise programs, patient/family education, and documentation including OASIS and billing
• Must be COVID vaccinated or have exemption on file
• Must have flu shot or declination, with mandatory mask wearing at all times if declined

• Home health setting providing skilled visits to patients across Pinellas County
• Patient population includes med/surg, cardiac, orthopedic (hips/knees/shoulders), post-surgical, COPD, CHF, pneumonia, CVA, falls, weakness, Parkinson's, failure to thrive, respiratory, neuro, trachs, trauma, AKA/BKA, and ADL-focused cases
• Serves Medicare, Medicare Advantage, and commercial insurance patients
• Completes 6-8 home skilled visits per day (minimum 6 points, may increase by manager discretion)
• 1-2 weekends required per contract
• Day shift schedule, 5x8-hour shifts from 8:00 AM to 5:00 PM
• Visits and hours are self-scheduled based on office needs
• Uses Bay Care Forms, Bay Care Scheduling, and Ace Timesheet documentation systems
  
  Provided with a laptop, MiFi for internet access, and portable printer
• Responsibilities include assessment, treatment planning, and education for patients with orthopedic, neurologic, and cardiopulmonary conditions
• Duties include recognizing and reporting signs and symptoms of physical, sexual, or psychological abuse
• Focus on safety and injury prevention throughout home visits
• Any solid color scrubs are acceptable attire
• Smoke-free facility policy in effect
• First-time travelers are not required to have prior travel assignment experience
