About the Role As a Home Health Licensed Practical Nurse, you will provide skilled nursing services to patients in their homes under the supervision of a Registered Nurse and in accordance with physician orders and Medicare guidelines.

Key Responsibilities Provide skilled nursing care in the home under RN supervision

Perform patient assessments and report changes in condition to the supervising RN Administer medications and treatments as ordered

Provide wound care, catheter care, and other skilled nursing services within scope

Educate patients and caregivers on medications, disease processes, and safety

Accurately complete clinical documentation in the EMR in a timely manner

Collaborate with RNs, physicians, therapists, and interdisciplinary team members

Follow all Medicare, CHAP, and agency policies and procedures Qualifications Active LPN license in the state of Florida (required)
Home health or Medicare experience preferred

Experience using Wellsky/Kinnser Preferred Current CPR certification

Valid driver’s license, reliable transportation, and active auto insurance

Strong clinical and communication skills

Ability to work independently while remaining connected to a care team
