Overview
Home Health Registered Nurse. We are seeking a dedicated and experienced Home Health Registered Nurse to provide skilled nursing care to patients in their homes. This role involves delivering comprehensive care to adult and geriatric patients with a variety of medical conditions, including cardiopulmonary issues, post-operative needs, and other general medical diagnoses. The ideal candidate will have a strong clinical background, excellent IV and wound care skills, and proficiency in OASIS documentation.
As a Home Health RN, you will be responsible for conducting thorough patient assessments, developing and implementing individualized care plans, and providing skilled nursing interventions. You will work autonomously in patients homes, providing education and support to patients and their families. The position requires a valid driver s license and the ability to travel to multiple patient homes daily, with an average of 6-8 visits per day.
- Perform comprehensive patient assessments and develop individualized care plans.
- Administer medications and treatments as prescribed by physicians.
- Provide wound care, including advanced wound management techniques such as wound vac dressing changes and ABI compression dressings.
- Perform venipuncture and administer SQ/IM injections.
- Manage and care for various devices, including PICC lines, Midlines, SQ ports, Pleurx drains, and Foley catheters.
- Educate patients and families on disease processes, medication management, and self-care techniques.
- Document all patient interactions and care provided in a timely and accurate manner using the designated electronic health record system (Bay Care Forms, Bay Care Scheduling, Ace Timesheet).
- Collaborate with the interdisciplinary team, including physical therapists, social workers, and pharmacists, to ensure coordinated patient care.
- Perform cardiopulmonary assessments and provide post-operative care.
- Perform staple and suture removal.
- Minimum of 1 year of recent experience as a Registered Nurse.
- Minimum of 1 year of Home Health experience with OASIS training and experience.
- Must have valid driver s license and car for home visits.
- Must be COVID vaccinated or have an approved exemption form on file.
- Must have flu shot or declination form and wear a mask at all times while at work.
- Recent patient care experience within the last 3 months.
- Cerner charting experience.
- Proficiency in infusion therapy.
- Strong IV and phlebotomy skills.
- Expertise in wound care management, including pressure ulcer prevention and staging.
- Knowledge of general medical diagnoses and high-risk diagnoses such as HF, COPD, MI, Pneumonia, and Stroke.
- Ability to perform cardiopulmonary assessments.
- Competent in the use of glucometers and pro-time machines.
- Proficiency in clinical assessment documentation.
- Knowledge of Medicare, Medicare Advantage, and Commercial insurance guidelines for home health.
- Ability to perform admission assessments, recertifications, resumption of care, and start of care visits.
- Strong communication and interpersonal skills.
- Ability to work independently and manage time effectively.
