Chapters Health is seeking a skilled RN Case Manager to assess patient and family needs, coordinate the Plan of Care with the Interdisciplinary Team, and provide clinical, palliative and supportive care to keep patients in their home environment whenever possible.
The role requires an active RN license in Florida, at least one year of nursing experience (hospice or hospital preferred), and strong communication skills for collaboration with physicians and IDT members.
