Job Summary:

Provides specialized case management nursing services to patients within a hospital setting, coordinating care to ensure optimal health outcomes. Acts as a linchpin in discharge planning and resource utilization, supporting patient recovery and efficient care delivery.

Responsibilities:

• Evaluate patient needs and develop individualized care plans in collaboration with medical teams.
• Coordinate with multidisciplinary staff to facilitate timely and safe discharges.
• Monitor patient progress, adjusting care plans as necessary to meet clinical goals.
• Document clinical activities and patient interactions accurately using the electronic medical record (EMR) systems.
• Educate patients and families on post-discharge care, resources, and support services.
• Advocate for patients’ needs to optimize their care experience and outcomes.

Qualifications:

• Maintain valid RN licensure in the state of Florida.
• Demonstrate at least 1 year of recent hospital case management experience.
• Possess proficiency in Midas and Meditech EMR systems.
• Exhibit strong clinical assessment, critical thinking, and communication skills.
• Show ability to manage complex discharge planning and patient coordination.
• Uphold a professional, adaptable, and compassionate demeanor.
