Lutheran Services Florida (LSF) seeks a Case Manager Support Specialist to help deliver compassionate care to vulnerable children and families. In this role, you'll assist case managers with intakes, documentation, and coordination of services across child welfare, behavioral health, refugee, and family support programs. You will manage client records in electronic systems, schedule appointments, and help connect families to healthcare and community resources.

Working in a mission-driven, collaborative environment, you'll support timely, accurate service delivery while upholding LSF's values of integrity, diversity, and respect.

Responsibilities

  • Support case managers with intakes, assessments, and service coordination for children and families.
  • Maintain accurate client records in EHR and databases, ensuring documentation meets agency and regulatory standards.
  • Schedule appointments, follow-ups, and referrals to healthcare, behavioral health, and community resources.
  • Communicate with clients, caregivers, and providers to gather information and support care plans.
  • Assist with data collection and reporting for program outcomes and compliance.
  • Educate clients on available services, eligibility, and processes in a clear, compassionate manner.
  • Coordinate transportation, reminders, and logistics to reduce barriers to care.
  • Protect client confidentiality and follow HIPAA, agency, and funder requirements.
  • Collaborate with a multidisciplinary team to support individualized service and safety plans.
  • Participate in trainings, team meetings, and quality improvement initiatives.
