The RoleWe’re seeking a mission-driven Clinical Case Manager, LCSW – Older Adults to join our Complex Care team, a collaborative, multidisciplinary group dedicated to improving outcomes for older adults with complex medical, behavioral health, and psychosocial needs.
In this role, you’ll support a panel of patients using a whole-person, patient-centered approach—blending clinical case management with behavioral health interventions. You’ll partner closely with primary care providers, nurses, behavioral health clinicians, and community partners to help patients navigate a complex healthcare system and overcome barriers to wellness.
You’ll report to the Director of Complex Care and be a trusted voice in shaping how we deliver thoughtful, effective care to older adults.

- Has strong organizational skills and can manage competing priorities with care.
- Has experience with (or strong familiarity with) housing resources and navigation in Marin County (a big plus!).
- Manage a caseload of 30–40 Complex Care patients.
- Develop comprehensive, collaborative care plans with patients.
- Coordinate medical, behavioral health, and social services.
- Provide targeted behavioral health support and interventions.
- Address social determinants of health (SDOH) and other barriers to care.
- Connect patients to community resources and supports.
- Conduct home and community visits as needed.
- Triage and respond to care needs in a timely, thoughtful manner.
- Collaborate with an interdisciplinary care team (primary care, nursing, behavioral health, care navigation, and external partners).
- Participate in case conferences, trainings, and administrative meetings.
- Document and report data accurately and on time.
- Support satellite clinics and work occasional extended clinic hours when needed.
- Share insights to improve case management services and quality of care.
- Travel between clinic sites or community locations as required.
- Combination of clinic, community, and desk-based work.
- Prolonged standing, walking, and sitting.
- Occasional lifting (up to 15 lbs).
- Driving between clinic sites as needed.
- Compliance with immunization and fit-for-duty requirements.

- Licensed Clinical Social Worker (LCSW) with an active California license.
- Fluency in English (written and verbal).
- Computer literacy, including Microsoft Office and internet research.
- Ability to write clear, professional documentation.
- Valid California Driver’s License.
- 1–2 years of experience working with individuals with complex medical, behavioral health, and/or substance use needs.
- Experience working with older adult populations.
- Knowledge of legal, ethical, and regulatory standards related to patient rights and confidentiality.
- Bilingual Spanish skills.
