Schedule: this is a full-time role that will work 8-hour shifts from 8 am to 4:30 pm.

More About Our Team This team assists with discharge planning, ensuring patients receive the necessary resources for a smooth transition from the hospital. You'll coordinate with third-party payers, government programs, and community services to arrange appropriate care, housing, transportation, and other essential services. Your role will involve coordinating placement and support services, including long-term care, rehabilitation, behavioral/mental health, home care, transportation, and DME, as well as assisting with CPS, APS, adoptions, and end-of-life planning, including advance directives.

This position offers an opportunity to make a direct impact on patient care by helping navigate complex discharge processes and fostering continuity of care.

How You ll Contribute

A Case Manager Social Worker who excels in this role:

  • Interviews and assesses patients and/or patient s family, caregivers, and/or legal representatives.
  • Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.
  • Participates in case reviews to evaluate case management and progress. Consults with healthcare team members to promote, monitor, and evaluate compliance with patient s treatment plan.
  • Assists with discharge planning and processes. Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan.
  • Mentors new staff, students, and interns.
  • Performs Medicaid screenings as indicated for NHP.
  • Assists patients with Safety Net applications when needed.

What We re Looking For

Applicants should have a bachelor s degree in social work. Additional requirements include:

  • License:
    Licensed Social Worker within six months of hire/transfer date
  • Certifications:

    Basic Life Support (BLS) required 30 days from hire date
