This role focuses on outreach, engagement, care coordination, and treatment access and retention, helping clients build stability, independence, and sustained connection to care.

Services are delivered in the clinic, in the field, and at community or outreach locations. CCSS staff work collaboratively with interdisciplinary teams and community partners while maintaining the autonomy to build trusted, recovery‑oriented relationships with clients.

This Role Is a Good Fit If You:
  • Have experience providing community‑based behavioral health or recovery support
  • Are comfortable working with high‑acuity or underserved populations
, including individuals experiencing homelessness or justice involvement
  • Value harm reduction, person‑centered care, and recovery‑oriented practice
  • Can manage time, priorities, and documentation independently
  • Want work that balances structure with real‑world flexibility
Key Responsibilities:
  • Provide Comprehensive Community Support Services aligned with NMHCA/BHSD standards
  • Support clients with navigation, engagement, and follow‑through across behavioral health, medical, and social service systems
  • Address social drivers of health, such as housing stability, income supports, transportation, and life skills for independent living
  • Participate in interdisciplinary team collaboration and community partnerships
  • Maintain timely, accurate documentation in the electronic health record
  • Deliver services in clinic, community, outreach, and field‑based settings as appropriate
Qualifications:
  • Bachelor’s degree in a health‑related or human services field
  • Experience in community support, case management, outreach, or behavioral health services
  • Spanish‑speaking competency strongly preferred
  • State credentials as a Community Support Worker (CSW) or Certified Peer Support Worker (CPSW) are a plus
  • Applicants should already meet—or be able to quickly meet—New Mexico requirements to provide Comprehensive Community Support Services (CCSS) under Medicaid/Turquoise Care
Career Path & Growth

This role offers a clear pathway for professional growth within community and behavioral health services. CCSS staff may develop specialized focus areas such as housing stability, harm reduction, medication‑assisted treatment support, or care coordination for specific funded programs. With experience, training, and supervision, staff can grow into lead CCSS roles, program coordinator positions, or pursue further licensure and advancement within integrated care teams.
