We are seeking a compassionate and motivated Social Worker / Family Therapist (SW/FT) to join our Early Intervention (EI) team. In this role, you will support families of infants and toddlers by identifying needs, coordinating services, and providing counseling and advocacy aligned with Individualized Family Service Plans (IFSPs) and New Mexico FIT Program guidelines.
You will partner closely with families and multidisciplinary providers to help children reach developmental milestones while strengthening family supports.
Key Responsibilities- Assess family needs and develop individualized support plans aligned with IFSP goals
- Provide counseling, guidance, and family-centered coaching during EI visits
- Coordinate services and refer families to community resources as appropriate
- Monitor progress and adjust support plans per NM FIT program guidelines
- Meet EI documentation, benchmark, and billing deadlines
- Participate in Transdisciplinary Team Consultation Meetings (TTCM)
- Collaborate with EI providers and multidisciplinary team members
- Provide supervision and support to other therapists when needed
- Maintain accurate, timely documentation of all services provided
- Uphold ethical, culturally responsive, and family-centered practices
- Master’s degree or higher in Social Work or Family Therapy from an accredited program
- Experience working with children and families (birth to three preferred)
- Strong communication, organization, and interpersonal skills
- Ability to meet EI documentation and benchmark requirements
- Proficiency with documentation software and computer systems

This role is primarily home and community-based, requiring reliable transportation and the ability to travel locally. Compliance with Aprendamos health and safety requirements is required.
