Position: "Hybrid" Chronic Care Specialist - (44-00)

$16.37 to $22.00

Job Summary:

Under the supervision of the Director of Population Health, the Chronic Care Specialist is responsible for successfully supporting patients with high-risk health conditions to navigate the healthcare system. The Chronic Care Specialist interacts and collaborates with multidisciplinary care teams, to include physicians, nurses, pharmacists, laboratory technologists, social workers, and other educators to support the chronic care management/transition of care process. The Chronic Care Specialist acts as a resource for clinic staff.

The Chronic Care Specialist works in a less structured, self-directed environment and performs all nursing duties within the scope of an RN/LVN/LPN license of the applicable state board of nursing.

Core Competencies:

* Experience related to patient education and/or motivational interviewing skills and self management goal setting.

* Experience with outbound patient telecommunications

* Bilingual:
English and Spanish preferred

* Excellent verbal and written skills.

* Solid interpersonal skills.

* Ability to interact productively with individuals and with multidisciplinary teams.

* Excellent organizational and prioritization skills.

* Ability to travel throughout the assigned region and comfort with conducting home visits as needed.

* Knowledge of chronic diseases, especially COPD/asthma, diabetes, CHF, HTN, and CRF

* Proficient computer skills to work efficiently with electronic medical records.

Job Requirements:

* Licensed Practical/Vocational Nurse or other credentialed health professional as approved

* With an active and unrestricted NM or Compact State License

* Current BLS certification

* 2+ years of experience in a physician's office, clinical or hospital setting
