Salary: $16.37 to $22.00 per hour
Job SummaryUnder the supervision of the Director of Population Health, the Chronic Care Specialist is responsible for successfully supporting patients with high-risk health conditions to navigate the healthcare system. The Specialist collaborates with multidisciplinary care teams including physicians, nurses, pharmacists, laboratory technologists, social workers, and educators to support chronic care management and transition of care processes. The Specialist also serves as a resource for clinic staff and works in a less structured, self‑directed environment while performing all nursing duties within the scope of an RN, LVN, or LPN license in the applicable state.

- Experience with patient education and/or motivational interviewing skills and self‑management goal setting.
- Experience with outbound patient telecommunications.
- Bilingual:
English and Spanish preferred. - Excellent verbal and written skills.
- Solid interpersonal skills.
- Ability to interact productively with individuals and multidisciplinary teams.
- Excellent organizational and prioritization skills.
- Ability to travel throughout the assigned region and comfort with conducting home visits as needed.
- Knowledge of chronic diseases, especially COPD/asthma, diabetes, CHF, HTN, and CRF.
- Proficient computer skills to work efficiently with electronic medical records.
- Licensed Practical/Vocational Nurse or other credentialed health professional as approved.
- Active and unrestricted New Mexico or Compact State License.
- Current BLS certification.
- 2+ years of experience in a physician's office, clinical or hospital setting.
