At Tender Care Home Health & Hospice, is immediately seeking Full-Time and PRN LVN for our Las Cruces, NM location. We pride ourselves on providing compassionate, patient-centered care that elevates the quality of life for our patients and their families.
Job Responsibilities- Provides direct patient care as defined in State’s Name Nurse Practice Act
- Implements plan of care initiated by the registered nurse.
- Provides accurate and timely documentation consistent with the plan of care.
- Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care.
- Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinical Supervisor.
- Uses equipment and supplies effectively and efficiently.
- Participates in personal and professional growth and development.
- Performs other duties as assigned by the registered nurse.
- Maintains verbal and/or written communications among other personnel involved in the care of the patient; maintains clinical records on all patients referred to physical therapy and in consultation with the licensed physical therapist.
- Performs and submits required documentation of patient’s clinical notes, progress reports, summaries and case conferences along with changes in orders. All patient visit notes must be turned in the next scheduled work day, not to exceed forty-eight (48) hours.
- Advances professional growth; participates in training sessions at least 2 times annually; attends short courses, and studies professional journals and texts.
- Licensed Vocational Nurse in State where they are working.
- Should have good communication skills.
- CPR certification current.
- Valid driver license.
- Ability to work well with others, function well under pressure, and function independently.
- Must comply with the standards outlined in the Nurse Practice Act.
- Acts only with the scope of their state license or state certification or registration.
- LVN must keep their qualifications current at all times.
- Current State License to practice in New Mexicoas a Licensed Vocational/Practical Nurse.
- Language:
Bilingual English/Spanish preferred.
