RN FT - Nights

The Registered Nurse is responsible for providing nursing care to clients. Supervising care treatment and services of client care provided by LPNs, CNAs and other staff on assigned unit and shift. Developing, implementing, and evaluating an individual plan of care for each client. Assessment and re-assessment of clients. Communicating appropriate information with the interdisciplinary team. Reporting client concerns and changes in condition to supervisor and/or provider in a timely manner.

Roles and Responsibilities

Essential functions include:

  • Providing and coordinating care by assessing physical and behavioral health needs of client, developing and implementing nursing care plans, maintaining medical records and educating clients and their families about various physical and behavioral health conditions while ensuring client safety at all times.
  • Assessing clients' status on an ongoing basis and communicating findings to providers timely.
  • Ensuring nursing treatment plans are specific to assessed needs and initial treatment plan is completed upon admission.
  • Obtaining provider's orders (verbal or via telephone) accurately and documenting appropriately; transcribing and reviewing provider's orders ensuring all required elements are present and appropriate per policy and procedures.
  • Accurately administering medications, per ordered, utilizing at least two client identifiers.
  • Notifying providers of significant changes in status such as difficulties with detox, medical emergencies, pertinent lab results, or change in medical status regarding response to medications, etc.
  • Assessing and documenting effects of PRN medications.
  • Maintaining accountability of administration, recording, control and security of medications, including narcotics.
  • Completing admission and discharge processes efficiently and accurately as directed.
  • Conferring regularly with treatment team (e.g., nurses, therapists, program managers, provider) regarding client care. Attending and participating in the unit activities such as therapy groups, family meetings as appropriate, staffing, unit meetings, etc.
  • Providing education/explanation to clients, families, and co-workers regarding medication usage/maintenance, common side effects, client's rights, as well as other topics necessary for optimal client care.
  • Monitoring client acuity, census, staffing needs, and escalating coverage needs to supervisor/Charge RN.
  • Ensuring thorough, accurate documentation is completed for all clients on each shift and client assignment sheets are completed daily and tasks are carried out as assigned.
  • Communicating thorough hand-off reports to oncoming staff and supervisors.
Education/Experience/Skill Requirements

Requirements include:

  • Graduation from an approved/accredited school of nursing.
  • One or more years' nursing experience within clinical area of service provided by facility preferred.
  • Experience in behavioral health/psychiatric/addiction recovery environment preferred.
  • Knowledge of and experience with psychotropic medications and medication administration procedures.
  • Ability to apply nursing standards and practice within an acute psychiatric setting.
Licenses/Designations/Certifications

Required certifications include:

  • Valid current RN license from the state or from a compact state, where applicable.
  • Certified in Psychiatric Nursing preferred.
  • CPR and de-escalation and restraint certification required (training available upon hire and offered by facility).
  • First aid may be required based on state or facility requirements.

This position is onsite at the facility and is not a remote position.
