About the RoleSierra Health Care is looking for a Clinical Manager to lead home health clinical operations in Las Cruces. In this role, you'll oversee a team of nurses and therapists, guide individualized patient care planning, and make sure every patient receives safe, coordinated, and compassionate care. Key Responsibilities- Supervise and support nursing and clinical staff to ensure safe, patient-centered care delivery
- Recruit, orient, and evaluate clinical staff, including ongoing competency validation
- Oversee individualized patient care plans and lead care conferences
- Coordinate closely with physicians, community partners, and the interdisciplinary team
- Lead quality and compliance efforts through QAPI, infection prevention, and emergency preparedness
- Ensure timely, accurate clinical documentation in the EMR
- Stay available for consultation and on-call coverage to support continuity of care
- Manage clinical staffing, scheduling, and resources to keep operations running smoothly
Requirements
What We're Looking For- Graduate of an accredited nursing or therapy program; current, unrestricted New Mexico license required
- Minimum 3-5 years of clinical experience, including at least 1 year in home health or a related setting
- Demonstrated ability to supervise clinical staff, coordinate care, and maintain regulatory compliance
- Strong knowledge of Medicare and Medicaid Conditions of Participation and state licensure standards
- Proven leadership, communication, and organizational skills
- EMR proficiency and comfort managing multiple clinical priorities
- Valid driver's license and reliable transportation
