We are seeking a dedicated and organized Family Service Coordinator (FSC) to join our Early Intervention (EI) team serving families across Doña Ana County. In this role, you will act as the primary point of contact for families, ensuring they receive the services and support needed to help their children reach developmental milestones.
The FSC plays a key role in coordinating care, managing caseloads, and facilitating communication between families and multidisciplinary providers in alignment with Individualized Family Service Plans (IFSPs) and New Mexico FIT Program guidelines.
Key Responsibilities- Manage a caseload of families to ensure timely and appropriate delivery of EI services
- Coordinate and facilitate IFSP meetings, service planning, and ongoing case management
- Maintain accurate and timely documentation in compliance with EI program requirements
- Conduct face-to-face meetings with families and providers to support service delivery
- Serve as a liaison between families, providers, and community resources
- Monitor service plans and adjust coordination as needed based on family needs
- Utilize critical thinking and decision-making to support families effectively
- Communicate clearly and consistently with all stakeholders involved in care
- Ensure compliance with all program guidelines and deadlines
- Bachelor’s degree or higher in Social Work, Early Childhood Education/Development, Family Studies, or a related field
- Strong organizational, communication, and interpersonal skills
- Detail-oriented with strong follow-through and customer service mindset
- Ability to manage multiple priorities in a dynamic environment

This position is in-person and community-based, requiring travel within Doña Ana County. Reliable transportation is required.
