Position: Home Care Service Coordinator (Bilingual - Scheduler)

Full Time Service Coordinator – Albuquerque, NM

Ambercare is seeking a full‑time Service Coordinator to schedule and supervise in‑home care workers and clients in a geographic area. Responsibilities include conducting in‑home visits, monitoring and reporting quality of services, and ensuring care plans meet client needs.

Schedule:

Monday through Friday, 8:00 AM – 5:00 PM.

Responsibilities
  • Schedule home care workers in the system; manage changes due to client requests, illness, vacation or leaves of absence.
  • Provide alternate coverage to maintain care plan compliance.
  • Contact care providers and clients to provide service updates.
  • Monitor clients’ condition and Home Care Aides’ performance.
  • Maintain all company and Department on Aging rules, regulations, and standards.
  • Conduct home visits to welcome new clients and review the Welcome Packet.
  • Observe and evaluate Home Care Aides in the client’s home as directed by Supervisor to ensure Care Plans are followed.
  • Prepare and complete accurate evaluation reports and case notes.
  • Maintain schedules to ensure timely completion of all assigned home visits.
  • Attend weekly branch meetings.
  • Maintain a high degree of confidentiality regarding sensitive information.
  • Maintain regular, predictable, consistent attendance and flexibility to meet the department’s needs.
  • Follow Medicare, Medicaid, and HIPAA regulations and requirements.
  • Abide by all regulations, policies, procedures and standards.
  • Perform other duties as assigned.
Qualifications
  • Minimum of 2 years’ experience working in senior services or related social service field.
  • Excellent oral and written communication skills.
  • Strong interpersonal skills.
  • Ability to prioritize and handle high call volumes.
  • Client-focused and customer‑service driven.
  • Proficient with Microsoft Word, Excel, and Outlook.
  • Reliable transportation, valid driver’s license, and insurance for travel within the branch service area.
