* This is a full-time, in-office position offering a Monday-Friday schedule! As the Administrative Assistant, you will serve as a trusted resource of the interdisciplinary team to support patient admissions, intake coordination, medical records management, and day-to-day office operations. Your role is essential to ensuring patients and families receive timely, compassionate service throughout their care journey.

Your ImpactServe as the primary point of contact for patients, families, referral partners, physicians, and care team members, delivering compassionate and professional service. Provide administrative support including answering phones, scheduling, managing correspondence, and data entry. Coordinate and manage patient referrals from receipt through admission. Verify patient demographics, insurance information, and physician orders to ensure accurate admissions. Support intake and clinical teams during periods of increased referral volume and census growth.
Maintain patient medical records and charts in compliance with agency policies and regulatory requirements. Prepare, scan, upload, and organize documentation within electronic medical records. Track intake activity, census reports, and key operational metrics. Monitor clinical and office supply inventory, coordinating orders to ensure uninterrupted patient care. Assemble admission packets and ensure all required documentation is accurate, complete, and organized. Create and coordinate patient welcome gift baskets that reflect our commitment to comfort and support.
Maintain inventory levels of admission materials and gift basket supplies to ensure readiness for new admissions. Support special projects and operational initiatives assigned by the Director of Operations.
- High school diploma or equivalent required.
- Previous Home Health or Hospice intake experience required.
- Strong communication, customer service, and relationship-building skills.
- Ability to multitask, prioritize responsibilities, and remain organized in a fast-paced environment.
- Professional, positive, and compassionate approach when interacting with patients, families, and staff.
