Currently, we are looking for a full‑time Scheduling Coordinator & Patient Care Coordinator/Physical Therapy Technician floater for our Smith Therapy Partners locations. Must be flexible to assist at any of our 11 locations with open availability 7:00 am – 7:00 pm Monday – Friday to work any schedule needed.
Patient Care Coordinator Essential Position Duties- Greeting warmly and maintain a friendly, welcoming relationship with all patient care coordinators, and any external connections.
- Clearly communicate with all staff, including clinical and corporate employees, in a friendly and courteous manner.
- Answer phones and greet patients with the utmost professionalism and care.
- Work closely with Billing Department, Physical Therapists and Aide staff to assure smooth patient flow.
- Maintain patient confidence and protect operations by keeping information confidential.
- Participate in and adhere to, as required, the HIPAA Policies and Procedures of Therapy Partner Group.
- Review Copay collection reports and help identify accounts that may need to be reviewed.
- Audit Journeys/Dashboard to ensure they are being worked on in a timely manner.
- Audit Payer Direct Portal to ensure notes are being uploaded/AUTH’s pulled.
- Check FOC Chats to ensure messages are being responded to and worked on.
- Chart audit day/s in advance to ensure we have charts with documents and AUTH’s as required.
- The Scheduler is responsible for scheduling referral appointments for Initial Evaluation and following visits, rescheduling, and canceling appointments as applicable, and sometimes quick verification of patient's demographic information.
- Sort every inbound referral and eFax to the appropriate subfolder within 24 hours to allow scheduling of every referral appointment within 24–48 hours.
- Work referral stages for the purpose of scheduling Initial Evaluations for new patients.
- Handle inbound call volume as well as place outbound calls.
- Analyze incoming referrals to obtain all demographic data necessary to schedule an initial appointment.
- Understand insurance benefits and authorizations for obtaining prior to scheduling if required.
- Collaborate and communicate effectively with patients to provide appointment time and date for Initial Evaluation and the 5 following visits.
- Demonstrate thorough knowledge of problem‑solving methodologies.
- Show and practice strong teamwork capabilities.

- Exercise time management, timely decision making, and conflict management.
- Prepare treatment room for patient by following prescribed procedures and protocols.
- Prepare patients for physical therapy treatment by welcoming, comforting, providing and/or assisting patient into physical therapy apparel (shorts, gowns, etc.).
- Do laundry, maintaining an ample supply of clean linens.
- Under direct supervision and with co‑signature by, record daily notes.
