Title: Ryan White Care Coordinator

Job PurposeThe Care Coordinator improves health outcomes for people living with HIV/AIDS by enrolling clients into Ryan White programs, determining payer sources for ADAP services, and ensuring compliance with program guidelines. The role involves advocacy, eligibility screening, data entry, documentation, and coordination with clients, agencies, and providers to ensure a seamless continuum of care.
Key Responsibilities Program & Eligibility Coordination- Maintain strong knowledge of ADAP, NMAP dates, and payer-of-last-resort rules.
- Review eligibility applications and verify required documentation.
- Enter and mirror referral data from Ryan White Part A and Part B into CAREWare and Salesforce within required time frames.
- Track and determine ADAP payer sources.
- Process interim eligibility updates and collect necessary proof documents.
- Manage a caseload of diverse HIV-positive clients with professionalism and compassion.
- Assist clients with obtaining EIVF and refer to HIP/CSAP as needed for insurance enrollment, premium support, tax assistance, and other services.
- Follow up with clients and partner agencies for missing or updated documentation.
- Respond to all client communication within 48 hours.
- Enter lab results into CAREWare.
- Run emergency referral reports twice daily.
- Submit accurate program reports on time.
- Handle protected health information in compliance with HIPAA.
- Perform general administrative duties as needed.
- Field inquiry calls and explain Ryan White program requirements.
- Maintain familiarity with AHN services and other healthcare programs (Medicaid, Medicare, Nevada Check-Up, ACA).
- Participate in duties related to state and county grant requirements.
- Flexibility with department scheduling.
- Ability to complete copying, faxing, and large-scale mailings.
- Must pass Nevada background check and Medicare/Medicaid fraud screening.
- Obtain Community Health Worker certification within one year (agency-funded).
- May be required to obtain Exchange Enrollment Facilitator License (EEF).
- High school diploma required; AA degree preferred.
- One year of experience in healthcare-related social work, medical billing, or patient coordination preferred.
- One year of general office experience.
- Two years of experience using databases and Microsoft Office (Word, Excel, PowerPoint, Visio).
- Strong organizational, communication, and customer service skills.
- Ability to sit 6-8 hours, stand 3-6 hours, perform repetitive hand motions, and lift up to 30 lbs.
- Valid driver's license and ability to drive AHN vehicle.
- Spanish bilingual required.
This position is funded through external grant sources. Continued employment is contingent upon the ongoing availability of this funding. If funding is reduced or discontinued, the position may be changed or eliminated.
Work EnvironmentModerate noise level; primarily a climate‑controlled office.
