Since 2015, Nevada Behavioral Health Systems (NBH) has been providing mental health and substance abuse services to residents across Nevada. By collaborating with a diverse network of experienced behavioral healthcare providers, we have built a community‑based care system that offers members personalized options tailored to their individual needs. In 2023, we rebranded as Novum Health with the expansion of our services in Arizona, California, and Utah.

Location

Pahrump, Nevada – Onsite (This role is not eligible for Remote or Hybrid)

Starting Salary Range

$23 per hour (This is a reasonable estimate and final offers will be based on experience, education, certification, skills, and internal pay equity.)

What’s the role?

Community Health Workers (CHWs) at Novum Health are trusted members of the community who help individuals navigate challenges with mental health, substance use, and social barriers. CHWs meet people where they are, whether at home, in the community, or in recovery settings. Building trust, providing support, and connecting members with care and resources.

If you have experience in behavioral health, peer support, or recovery communities, this role gives you the opportunity to make a direct impact by helping individuals access the services they need and overcome obstacles on their journey to wellness.

What you’ll do
  • Assessing the healthcare needs of individuals within the community and identifying gaps in care.
  • Collaborating with healthcare providers, social service agencies, and community organizations to develop comprehensive care plans for patients.
  • Assisting patients in navigating the healthcare system and accessing necessary services, such as medical appointments, specialist referrals, diagnostic tests, and community resources.
  • Coordinating and scheduling appointments for patients with multiple healthcare providers to ensure continuity of care and optimal care coordination.
  • Facilitating communication between patients, providers, and community partners.
  • Monitoring patients' progress and adherence to care plans, providing support and education to promote self‑management and prevention.
  • Conducting home visits or outreach activities to assess patients' living conditions, social support systems, and adherence to treatment plans.
  • Advocating for patients' needs and rights within the healthcare system and community.
  • Documenting patient interactions, care plans, and progress notes in electronic health record and care management systems.
  • Participating in interdisciplinary team meetings and case conferences to ensure effective coordination and collaboration with other healthcare professionals.
  • Providing transportation, with personal vehicle, for members to appointments when needed (mileage is reimbursed).
  • Additional duties as assigned.
  • Building strong relationships with individuals and families to promote trust and reduce stigma.
What you’ll bring
  • Comprehensive knowledge of healthcare systems, community resources, and social services available to support patient care.
  • Knowledge of behavioral health, substance use treatment, and community resources.
  • Strong interpersonal and communication skills to establish rapport with patients, families, and healthcare providers.
  • Excellent organizational and time management skills to manage multiple patients and coordinate care effectively.
  • Ability to assess patient needs, develop individualized care plans, and evaluate outcomes.
  • Proficiency in using electronic health records and care management software.
  • Understanding of privacy regulations and compliance requirements related to patient information and data security.
  • Compassionate and patient‑centered approach to care, with a commitment to advocating for vulnerable populations.
  • Problem‑solving and critical thinking skills to address barriers to care and resolve complex situations.
