Job Purpose
The Ryan White Non-Medical Case Manager supports clients living with HIV/AIDS by helping them access medical, social, and community services that improve health outcomes and quality of life. This position facilitates enrollment into Ryan White programs, coordinates referrals, assists with care navigation, and ensures continuity of support through accurate documentation, data entry, and compliance with all grant, reporting, and program requirements.
The Case Manager also provides education, advocacy, and problem-solving to help clients achieve viral suppression and overall stability.
- Case Management & Client Support
- Conduct case management assessments, screening tools, acuity tools, and develop individualized service plans.
- Support treatment adherence through education, care coordination, and regular follow-up.
- Manage a diverse caseload of clients living with HIV/AIDS with professionalism, compassion, and cultural sensitivity.
- Educate clients on AHN services, partner programs, and available community resources.
- Assist clients with enrollment in Ryan White programs, supportive services, HOPWA (when eligible), and other benefits that improve health and stability.
- Maintain client contact according to grant guidelines (follow-ups at 30 days, 60 days, and prior to service expiration; recertification every 6 months).
- Referral Coordination
- Place and track referrals for Part A, Part B, AHN internal programs, and community partners.
- Ensure all referrals and follow-up actions are completed by the end of each business day.
- Data Management & Reporting
- Enter case notes, services, referrals, and documentation into CAREWare A/B, Salesforce, and RWISE daily.
- Ensure all client files, events, and documentation are audit-ready and updated.
- Enter CD4 and Viral Load labs into CAREWare
A. - Meet all federal, state, and local reporting requirements, including deliverables for Nevada Department of Health and Clark County.
- Submit programmatic reports accurately and on time.
- Administrative & Program Support
- Maintain spreadsheets, schedule appointments, and assist with high call volume.
- Respond to calls, emails, and messages within 48 hours.
- Perform general administrative tasks such as copying, faxing, mailings, and phone system use.
- Participate in staff meetings, supervision, and ongoing training.
- Support continued development and improvement of the Ryan White Program.
- Adapt to changing program needs, duties, and schedules.
- Other duties as assigned.
- Spanish bilingual preferred.
- Bachelor's degree in social work, public health, psychology, sociology, or related field preferred; or equivalent combination of experience and education.
- Minimum one year in healthcare-related social work or patient/client coordination; OR two years of experience working with special-needs populations and case management knowledge.
- Minimum two years of computer experience, including Microsoft Office and database systems.
- Experience with CAREWare, Salesforce, or similar systems preferred.
- Strong organizational, communication, and customer service skills.
- Ability to multitask, think critically, and work well in a fast-paced environment.
- Compassionate, professional, and able to build strong relationships with clients and partner agencies.
- Knowledge of community social services;
Medicaid, Medicare, ACA, and Nevada Check-Up; and HIV/AIDS Drug Assistance Program (ADAP). - Must abide by all HIPAA requirements.
- Valid driver's license; ability to drive AHN company vehicle.
- Ability to lift/carry up to 30 lbs.
- Ability to sit 4-6 hours/day and stand 3-6 hours/day; frequent hand/wrist movement.
- Must pass Nevada background check and Medicare/Medicaid fraud screening.
- Must obtain Community Health Worker Certification within one year (agency-funded).
Work is performed in a climate-controlled office with moderate noise. The position requires extended computer use, in-person client interaction, and occasional lifting of supplies.
