She Recruits is seeking an experienced and highly motivated Director of Case Management to lead a large, high-acuity case management department within a major regional hospital. This role requires deep expertise in hospital discharge planning, interdisciplinary coordination, resource utilization, and regulatory compliance. The ideal candidate is a strong, strategic leader capable of driving performance, building physician relationships, and overseeing a complex case management structure across a large facility.

Role Overview:
The Director of Case Management maintains full responsibility for managing departmental activities, ensuring alignment with organizational policies and external regulatory requirements. This leader coordinates integration between patient care processes, discharge planning, social services, and post-acute partners to ensure continuity of care and optimal use of clinical resources. The Director is accountable for daily operations and key departmental outcomes and works closely with hospital leadership, physician teams, and community partners to elevate care coordination across the continuum.

Key Responsibilities:

• Lead, develop, and oversee the case management, social work, and chaplaincy departments to ensure safe, effective, patient-centered transitions of care

• Ensure staff adherence to regulatory requirements, organizational policies, and industry standards

• Build and maintain strong relationships with physicians, post-acute providers, and interdisciplinary teams to optimize patient flow and discharge planning

• Review departmental data, identify gaps, and independently develop action plans for performance improvement

• Coordinate care management services across hospital departments to ensure continuity of care and resource utilization

• Develop mechanisms to improve throughput, reduce avoidable delays, and support timely discharge processes

• Provide day-to-day operational oversight for a large hospital case management structure

• Engage frontline leadership teams, including managers and supervisors, to drive accountability, team development, and departmental alignment

• Participate in on-call rotation as needed based on volume and operational demands

Qualifications:

• Bachelor's degree required

• Master's degree in Nursing, Health Administration, or Business Administration preferred

• Must hold RN licensure, LCSW, or LMSW

• Minimum 3 years of acute care hospital case management experience required

• Minimum 2 years of case management leadership experience required

• Must bring extensive leadership experience with strong data review, action planning, and strategic relationship-building skills

• Must be a proactive, self-motivated leader with strong work ethic and high accountability standards
