About the Role

The Community Liaison serves as a key ambassador for Unified Care Hospice in the Las Vegas area. In this role, you will develop and nurture relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, and other referral partners.

You will work closely with our clinical and leadership teams to ensure referrals are supported with excellent communication, responsiveness, and care coordination.

Qualifications
  • 3+ years of hospice or home health business development experience
  • Experience developing and maintaining referral relationships
  • Familiarity with the Las Vegas healthcare community
  • Strong interpersonal and communication skills
  • Organized, dependable, and self-motivated
  • Commitment to ethical, patient-centered practices
Responsibilities
  • Build and maintain meaningful referral partnerships
  • Identify opportunities to strengthen community connections
  • Support admissions and census growth through relationship-based outreach
  • Represent Unified Care Hospice at community and networking events
  • Collaborate with clinical staff to ensure smooth transitions of care
  • Maintain accurate documentation and activity reporting
