General Purpose
The primary purpose of this position is to provide direct nursing care to residents and to supervise the day‑to‑day nursing activities performed by nursing assistants. Supervision must be in accordance with federal, state, and local standards, guidelines, and regulations governing the facility and may be directed by the Director of Nursing Services or Nurse Supervisor to ensure the highest quality of care.
AdministrativeFunctions
- Direct the day‑to‑day functions of nursing assistants in accordance with regulations governing the long‑term care facility.
- Ensure all nursing personnel comply with the written policies and procedures of the facility.
- Review and recommend revisions to department policies, procedure manuals, and job descriptions.
- Plan shifts’ services, programs, and activities with nursing staff and support personnel.
- Maintain the Nursing Service Procedures Manual to reflect current nursing procedures.
- Report shift activities as required and coordinate with resident services to maintain total care regimens.
- Ensure compliance with job descriptions and participate in developing the quality assurance program.
- Participate in facility surveys and inspections by authorized agencies.
- Review and update resident discharge plans and assist in planning the nursing services portion of discharge.
- Interpret departmental policies for personnel, residents, visitors, and agencies.
- Admit, transfer, and discharge residents and complete related documentation.
- Complete accident/incident reports and submit to the Administrator.
- Submit resident charge slips to the Business Office and maintain the Daily Census Report.
- Perform general administrative duties such as forms, reports, evaluations, and charting.
- Maintain confidentiality of system access credentials and report violations.
- Report unauthorized access attempts to the Administrator.
- File required recordkeeping forms upon admission, transfer, and discharge.
- Encourage physicians to review treatment plans and sign orders in accordance with policies.
- Record telephone orders and transcribe them to resident charts and medication cards.
- Chart nursing notes that reflect care provided and resident response.
- Submit accident/incident reports and other required documentation to the Director.
- Record diet changes, discrepancies in orders, and complete transfer forms.
- Ensure proper documentation of unauthorized discharges and perform routine charting duties.
- Sign and date all entries in the resident’s medical record.
- Prepare and administer medications per physician orders.
- Verify resident identity before medication administration.
- Prevent medication cross‑dose and maintain floor stock supplies.
- Order and manage narcotic records, report discrepancies, and review medication cards.
- Notify physicians of automatic stop orders and dispose of drugs per procedure.
- Participate in performance evaluations and staffing recommendations.
- Report absenteeism and staffing needs to the Nurse Supervisor.
- Develop work assignments and provide leadership to nursing personnel.
- Conduct daily rounds to ensure adherence to nursing standards and report problems.
- Ensure nursing assistants are trained and supervised appropriately.
- Maintain relationships with inter‑departmental staff and promote a positive work environment.
- Address complaints, grievances, and disciplinary actions fairly.
- Report occupational exposures and fraudulent incidents to the Administrator.
- Inform staff of new admissions and ready rooms.
- Greet and escort new residents, orient them and families.
- Coordinate with physicians and plan diagnostic services.
- Consult with physicians and review charts for treatments and medications.
- Implement nursing objectives, conduct periodic checks, and coordinate with social programs.
- Administer medical procedures, use restraints appropriately, and obtain specimens.
- Monitor resident health, maintain safety, and report issues to the Supervisor.
- Assist with transportation, discharge, and funeral arrangements.
