We are seeking a dedicated and reliable Licensed Vocational Nurse (LVN) to join our skilled nursing team. As an LVN in home health, you will provide direct patient care under the supervision of a Registered Nurse and in accordance with the plan of care. Visits may include wound care, medication administration, chronic disease monitoring, and patient/family education.

• Perform skilled nursing visits to patients in their homes in accordance with the care plan.
• Accurately document assessments, care provided, and patient response in EMR (Synergy experience preferred).
• Monitor and report changes in patient conditions to supervising RN and physician.
• Provide wound care, catheter care, injections, and medication management.
• Collaborate with interdisciplinary team to ensure coordinated, patient-centered care.
• Educate patients and caregivers to promote independence and safety.
• Comply with Medicare/Medicaid and state regulations.

• Current LVN license in the State of Nevada (unrestricted).
• One (1) year of clinical experience (home health preferred).
• Valid driver’s license
  , auto insurance, and reliable transportation.
• CPR Certification (BLS).
• Ability to work independently and manage time effectively.
• Proficient in electronic documentation (EMR experience preferred).
