In-Home Nurse Practitioner or Physician Assistant - Las Vegas

At Signify Health, part of CVS Health, we are looking for a Full‑Time Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations and help patients receive comprehensive care in the comfort of their own homes.

• Fulfill the role’s clinical responsibilities as specified by Signify Health.
• Be flexible to travel locally and within licensed states, and conduct virtual visits as required.
• Deliver Diagnostic & Preventive Services products such as spirometry, diabetic eye exams, and bone density screening when indicated.

Your primary workplace will be the private residence of care plan members. You must be comfortable and able to safely work in homes that may contain pets, secondhand smoke, or other airborne allergens.
