Licensed Practical Nurse Opportunity

An Exciting Opportunity with an established Agency! Advisa Care is seeking a compassionate, skilled, and experienced Licensed Practical Nurses in the Las Vegas and surrounding areas to join our team! This role is ideal for a nurse who is passionate about providing patient-centered care, supporting families, and delivering high-quality clinical service in the comfort of patients' homes.

You will be part of an interdisciplinary team dedicated to dignity, comfort, and holistic care for patients across the continuum of home health and end-of-life services.

Responsibilities

• Provide skilled nursing care to home health and hospice patients
• Monitor patient conditions and report changes to the RN/Clinical Supervisor
• Administer medications and treatments as ordered
• Perform wound care, catheter care, and other skilled nursing interventions
• Educate patients and families on care plans, disease processes, and comfort measures
• Accurately document all patient care and services
• Collaborate with interdisciplinary team members to ensure continuity of care
• Support patients and families with compassion, dignity, and respect
