RN Hospice Case Manager Float

About This Role

The RN Hospice Case Manager Float coordinates care and provides ongoing nursing assessments of patient and family needs, collaborating with interdisciplinary team members and attending physicians to develop and implement the patient's plan of care.

• Pay: $29 - $52 per hour
• Location: Las Vegas, NV

• Schedule: Full Time

• Provide initial and ongoing assessments of the impact of the terminal diagnosis on the patient's needs.
• Assess the ability of the caregiver to meet the patient's immediate needs upon admission and throughout care.
• Apply specific criteria for admission and re-certification to hospice care.
• Implement the individualized plan of care and recommend revisions as necessary.
• Consult and educate the patient/family and other caregivers regarding the disease process and end-of-life care.
• Initiate appropriate preventive and rehabilitative nursing procedures.
• Ensure continual assessment of assigned patient's and family's needs.
• Provide ongoing evaluation of the patient/family response to care.
• Supervise LPNs/LVNs, Hospice Aides, and other paraprofessionals.
• Document progress toward established goals as per standards.

• Education:
• BSN preferred
• Experience:
• 1+ years of hospice experience within the last three years or 1+ years of clinical experience in a hospital, home health, or community setting preferred.
