Nurse Practitioner

Job overview

The Nurse Practitioner contributes to AHF by monitoring patients and preparing comprehensive patient care plans at our Wellness Centers. With a friendly, helpful and compassionate disposition, the NP enhances the member experience, conducts diagnostic tests, evaluates lab reports, provides risk-reduction counseling for STIs, and interprets healthcare conditions.

• Conduct patient assessments
• Provide treatment and presumptive treatment for STIs
• Initiate and manage PrEP and PEP
• Review laboratory results for all clients, both new and returning
• Ensure rapid diagnosis, immediate linkage to care, and prevention of HIV
• Ensure timely clinical care, reduce wait times, and maintain continuity of services
