Position: RN Clinical Coordinator (Hospice Leadership)
Description
This is a clinical leadership position. The RN Clinical Coordinator supervises and provides direct patient care for the Adult Hospice Program. The position supervises all other members of the Interdisciplinary Group either directly or indirectly through their immediate manager. The Clinical Coordinator must be effective in explaining the hospice concept of care to patients, families, physicians, resource and referral sources and other community members.

Requirements
Essential Job Functions:
Provides direct leadership, management, and oversight of the organization's four key clinical service areas:
Triage, Admissions, Clinical Education, and Certified Nursing Assistants (CNAs), as assignedFollows an individualized Plan of Care established by the RN for each patient and covers patient visits and case management as neededParticipates in management on-call dutiesAssists with hiring, counsels, and administers progressive discipline to assigned staff members as neededAssists in developing plans of correction in response to surveys as neededEvaluates and conducts performance evaluations of staffEnsures proper staffing and scheduling for patientsSupervises and delegates duties to multidisciplinary clinical personnelAssists with coordination of Interdisciplinary Team MeetingsUnderstands/provides basic principles of palliative care/pain management within the Scope of PracticeParticipates in Performance Improvement activities as requestedDemonstrates reliability by making every attempt to report to work on time, providing proper notification for absence or tardiness, participating in required meetingsDemonstrates ability to solve problems and make decisions by assessing situations and considering alternatives, seeking guidance and direction as necessary, utilizing proper chain of commandDemonstrates effective time management skills by coordinating activities to achieve maximum productivity and efficiency; willingness to adjust or revise work schedules as requestedStays current with all mandatory agency and regulatory requirementsManages budgets, hires, and trains staff, and assists in developing and implementing policies and protocolsCompletes assigned projects within specified time framesOther duties, as assigned________________________________________________________________________________

Assists in creating and implementing policy/proceduresAttends scheduled staff meetings and/or in-service trainingParticipates and interacts in all complaint processes as directedEnsures that all regulatory standards are met by team, including State, Federal, CHAP and other regulatory bodiesImplements a delegation of responsibility for the availability of clinical services on a twenty-four (24) hour basisAdheres to patient rights, abuse reporting, and confidentiality policiesFosters and maintains an effective working relationship between peers, professional staff, and other personnelManages clinical aspects of patient careAssures patients are admitted under correct requirements for hospice serviceCoordinates staff meetings and/or in-service training as neededManages confidential informationAdheres to the departmental dress code requirements related to personal grooming and attireMaintains regular and consistent attendanceAssumes responsibility for ongoing personal development and continuing educationOther duties as assigned_____________________________________________________________________________
Physical/Safety Requirements:
Must have the strength and endurance required for standing, going up and down stairs, walking, lifting, bending, pushing, and pushing, on a consistent basis up to 50 lbs.Utilizes appropriate durable medical equipment and PPE as needed in a safe mannerUses appropriate body mechanics as needed for personal use and safe patient careCategory 1 for potential exposure to blood/body fluids.
