About the Role:

As a Registered Nurse MDS Coordinator, you will be responsible for coordinating and facilitating the Minimum Data Set (MDS) assessment process, ensuring accuracy, completeness, and timely submission to regulatory agencies. You will work closely with the interdisciplinary team to develop and implement comprehensive care plans, monitor resident progress, and identify areas for improvement.

Responsibilities:

  • Coordinate and facilitate the MDS assessment process, ensuring compliance with regulatory guidelines

  • Develop and implement comprehensive care plans in collaboration with the interdisciplinary team

  • Monitor resident progress, identify areas for improvement, and make recommendations for adjustments

  • Collaborate with healthcare professionals to ensure accurate and timely reporting of resident data

  • Participate in quality improvement initiatives and contribute to the development of policies and procedures

  • Maintain accurate and detailed records of resident assessments, care plans, and progress

Requirements:

  • Current Registered Nurse (RN) license in the state of Nevada

  • Minimum 5 year of experience in a long-term care or geriatric setting

  • Ability to work independently and as part of a team

  • Excellent communication, organizational, and problem-solving skills
