Position Overview:Med-Care Providers is seeking a compassionate and experienced Nurse Practitioner (APRN-CNP) to provide in-home primary care services to homebound Medicare and Medicaid patients throughout Las Vegas and surrounding areas. This role is primarily focused on house visits
, with the opportunity to also handle urgent care visits via telehealth as needed
.
This is a flexible position ideal for providers seeking full-time or 2–3 fixed days per week with a consistent schedule.
Responsibilities:- Conduct comprehensive home visits for elderly and homebound patients
- Evaluate and manage chronic and acute conditions
- Perform medication reconciliation, lab reviews, and coordinate with interdisciplinary teams
- Respond to urgent care needs via telehealth
, as scheduled or on-demand - Complete accurate and timely documentation to support compliance and billing
- Collaborate with care coordinators and specialists when needed
- Flexible:
Full-time or minimum of 2–3 fixed days/week - Primarily in-person house visits
, with telehealth urgent care as needed - Competitive per-visit or daily pay rate
- Active APRN-CNP license in Nevada
- Spanish Speaker
- Must be Medicare and Nevada Medicaid credentialed (or eligible)
- Minimum 1 year of experience in primary care, geriatrics, or urgent care
- Reliable transportation for home visits
- Tech-savvy and comfortable using telehealth platforms
- Bilingual (English/Spanish) preferred but not required
