The Registered Nurse at Inspirada Primary Care provides comprehensive clinical support within the primary care setting and serves as a key member of the clinic's care management and population health team. The RN is responsible for patient assessment, triage, education, chronic disease management, medication management, care coordination, transitional care, and clinical follow-up.
The position also supports the clinic's Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Behavioral Health programs, including patient identification, eligibility and benefits review, enrollment, monitoring, outreach, coordination, documentation, and escalation of clinical concerns.
The RN works closely with providers, front-desk/eligibility staff, billing/RCM, behavioral health professionals, specialists, pharmacies, and other healthcare partners to ensure patients receive appropriate and coordinated care.
Key Responsibilities- Provide primary care nursing, including assessment, triage, medication reconciliation, patient education, results review, and clinical follow-up.
- Manage RPM services, including patient identification, E&B review, enrollment, monitoring of physiologic data, patient outreach, and escalation of abnormal findings.
- Support CCM services through care planning, chronic disease monitoring, patient outreach, education, and coordination with providers and specialists.
- Coordinate behavioral health services, including E&B verification, referrals, follow-up, care-plan support, and escalation of urgent concerns.
- Collaborate with Eligibility & Benefits and RCM teams to verify coverage, benefits, authorizations, patient responsibility, and payer requirements for RPM, CCM, and behavioral health.
- Provide care coordination and transitional care, including post-hospital/ED follow-up, referrals, medication access, and identification of barriers to care.
- Monitor quality and population health outcomes, including care gaps, RPM/CCM engagement, clinical interventions, and program performance.
- Maintain accurate EHR documentation, support audit readiness, and ensure compliance with HIPAA, Nevada nursing requirements, payer guidelines, and clinic policies.
- Current Nevada Registered Nurse (RN) license in good standing.
- Bachelor's degree in Nursing.
- 2+ years of clinical nursing experience preferred.
- Primary care, ambulatory care, chronic disease management, case management, telehealth, population health, RPM, or CCM experience preferred.
- Experience with EHR systems; eClinical
Works experience preferred. - Knowledge of insurance eligibility and benefits preferred.
- Strong understanding of chronic conditions including hypertension, diabetes, CHF, COPD, obesity, and hyperlipidemia.
- Strong communication, clinical judgment, organization, and documentation skills.
- Ability to work independently and collaboratively with physicians and the multidisciplinary care team.
