Salary: $118,000 - $129,000 annually, based on experience

Productivity Bonus: $1,000 - $4,000 per quarter based on clinical productivity and performance

Position Summary

Reliant Health is seeking a compassionate, motivated, and clinically strong Nurse Practitioner to join our outpatient primary care team. The ideal candidate will provide comprehensive primary care services for adult and geriatric patients, including preventive care, wellness visits, chronic disease management, and acute care evaluations.

This position offers the opportunity to practice with autonomy while collaborating closely with physicians, hospitalists, specialists, care coordinators, and post-acute providers to ensure outstanding patient outcomes.

Essential Duties and Responsibilities

• Conduct comprehensive health assessments, including medical history, physical examinations, and preventative screenings
• Diagnose and manage acute and chronic medical conditions
• Develop individualized treatment plans based on evidence-based clinical guidelines
• Order, interpret, and communicate laboratory and diagnostic study results
• Prescribe medications and monitor therapeutic effectiveness
• Manage chronic disease populations, including hypertension, diabetes, COPD, heart failure, hyperlipidemia, depression, and other common primary care conditions
• Provide annual wellness visits and preventive health services
• Educate patients and families regarding diagnoses, medications, lifestyle modifications, and disease prevention strategies
• Coordinate care with specialists, hospitalists, therapists, home health agencies, and post-acute providers
• Ensure timely and accurate clinical documentation within the electronic medical record
• Participate in quality improvement initiatives and value-based care programs
• Promote preventive medicine and population health management strategies
• Maintain compliance with all regulatory, ethical, and organizational standards

Qualifications

Required

• Current Nevada APRN license in good standing
• Board Certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP)
• Current DEA registration
• Current BLS/CPR certification
• Strong clinical assessment and diagnostic skills
• Proficiency with electronic health record (EHR) systems
• Excellent communication and interpersonal skills
• Ability to function independently while collaborating effectively within a multidisciplinary team

Preferred

• Two or more years of outpatient primary care, family medicine, internal medicine, or community health experience
• Experience managing a medically complex adult and geriatric population
• Familiarity with value-based care and quality performance metrics
